They must have at least 50% of the vote at the time the Executioner contacts the GM.

For reasons of balance in this setup, the Mafia Roleblock is not performed by a named Mafia team member, no. It is a collective action which does not prevent the Mafia team members from carrying out other actions.

I am assuming that she is, yes, since she signed up to play. Confirmations are a nice way of reminding people the start of the game is nigh, but failure to confirm does not automatically mean withdrawal.
